Grievance Policy

We are committed to ensuring that your rights are protected.  If you feel that our staff has failed to follow our policies or has in any way denied you your rights, please follow these steps without fear of discrimination or reprisal

 

:​1.  Notify the Dignity & Hope Hospice and Palliative Care Chief Executive Officer at 1-866-201-8231, from 8:00am to 4:00pm, Monday through Friday.  You may submit your complaint in writing to 9245 N. Meridian Street, Suite 130, Indianapolis, IN  46260.  Most problems can be resolved at this level.

 

​2.  However, if you’ve already filed a complaint and aren’t satisfied with the response, or if you’re uncomfortable talking to your hospice provider, you may also contact the state's complaint hotline at 1-800-246-8909 (toll free) during state business hours.  If voicemail answers, please leave a brief message regarding your concerns along with your name and phone number, and your call will be returned.  The purpose of the hotline is to receive complaints or questions about care provided by any licensed or certified Indiana health care provider or supplier and to lodge complaints concerning the implementation of advance directive requirements.
